The Center for Reproductive Medicine and Robotic Surgery accepts many different insurance plans provided by the following insurance companies.  Some insurance plans offer “out-of-network” benefits so we still may be able to utilize your insurance benefits even if you do not see your insurance company listed.  Any applicable co-payment listed on your insurance card will be collected at each visit.

Note for Coventry Patients: Coventry requires a prior authorization for all infertility services if your plan offers coverage for diagnostic testing and/or treatment for infertility.  Please inform the office at the time your appointment is being scheduled that a prior authorization needs to be obtained.  The office will notify you if our request for prior authorization has been denied.
