Insurance Plans

The Center for Reproductive Medicine and Robotic Surgery accepts many different insurance plans provided by the following insurance companies.  Some insurance plans offer “out-of-network” benefits so we still may be able to utilize your insurance benefits even if you do not see your insurance company listed.  Any applicable co-payment listed on your insurance card will be collected at each visit.


Treatment Plans

At the Center for Reproductive Medicine and Robotic Surgery we offer patients without insurance coverage an all-inclusive payment plan which includes many of the services normally associated with an IVF (In Vitro Fertilization) Cycle. This will help the patient know the end costs up front instead of being surprised by add on costs. The following plan descriptions will help you understand exactly what is covered for each plan.

Online Payment

An online payment system is an Internet-based method of processing economic transactions. It allows a vendor to accept payments over the web or over other Internet connections, such as direct database connections between retail stores and their suppliers–a common method of maintaining just-in-time inventories.


At The Center for Reproductive Medicine and Robotic Surgery(CRMRS) we provide private, professional and compassionate fertility care. Dr. Jacob and our reproductive team provide a very personalized approach, from diagnostic assessment and early intervention through high-tech procedures like In Vitro Fertilization (IVF), and Intracytoplasmic Sperm Insertion (ICSI), intrauterine insemination (IUI) and Pre-Implantation Genetic Testing – Aneuploidy (PGT-A).
